I wanted to download PRE and Post Hospitalization Claim Form but only PRE Authorisation form is available on your website. Please mail me the Forms as I wish to submitmy Pre and Post Hospitlization Bills.I was hospitalised in Fortis Hospital, Noida from 18/02/2012 to 22/02/2012

you have so many ph.no. but no one to recevied.On friday my younger brother face this problem. we have your card no. NIC-GHI-161110-282074 he is highly injured & two days in I.C.U (dehradun).(are own cost) we don't know your registered hospital. now tell me the profit of your card.
